Before admission - an evaluation of whether a prospective resident is a safe, appropriate fit for your home, before you commit. Taking the wrong resident is expensive in every way: staff strain, care you can't bill for, and sometimes a discharge that hurts everyone involved.
When something changes - a nurse assessment when a resident moves in or their condition changes, so your documentation matches their real care needs. This is also what tends to reveal that you've been providing more care than you're being paid for.
What you get
A clear written assessment you can act on and keep in the file — my honest read on whether this placement works, what it would take, and where the risks are. If the answer is no, I'll say so. That answer is worth as much as a yes.
Common questions
How soon can you assess?
Usually within a few days. Placement decisions are often time-sensitive — say so when you call.
In person or virtual?
Either, depending on the resident. In-home, virtual, or telehealth.
Is travel extra?
No. Mileage is included anywhere in Washington.
Is a placement assessment the same as an LTC assessment?
They're the same service in practice - a registered nurse evaluating a resident's care needs. Whichever your case manager or referral source asks for, that's what I provide.
